Thrombocytopenia Initial Workup
Confirm that thrombocytopenia is real, identify bleeding or thrombosis requiring immediate action, then use the smear, clinical setting, medication exposure, and accompanying cytopenias to separate peripheral destruction from impaired production and urgent consumptive syndromes.

Identify presentations that cannot wait for a routine workup
Use clinical acuity and thrombosis status to set the pace of evaluation.
Separate stable isolated thrombocytopenia from acute thrombocytopenia in the emergency department or ICU. The latter may be the manifestation of life-threatening disorders, including thrombotic microangiopathies, and allows time for only a focused set of diagnostic tests before management decisions are required. Escalate immediately when thrombocytopenia accompanies thrombosis during or after heparin exposure. In acute coronary syndrome, the magnitude of platelet decline itself has prognostic importance: each 10% drop in platelet count was associated with increased mortality and bleeding risk. A smear is the fastest safeguard against an artifactual platelet count.
Review the peripheral blood smear for platelet clumping whenever thrombocytopenia is newly detected or clinically implausible.
