Immune Thrombocytopenia

Diagnose ITP only after confirming true isolated thrombocytopenia and excluding urgent thrombotic, marrow, infectious, drug-related, and pregnancy-specific causes. Management is driven by bleeding, platelet trajectory, treatment requirements, comorbidity, and durability goals rather than platelet count alone.

Clinical question: How should physicians confirm ITP, triage bleeding risk, and select initial and subsequent therapy?

Immediate decisions

Triage bleeding and exclude thrombocytopenic emergencies

Treat active critical bleeding while the diagnostic evaluation proceeds.

For intracranial, gastrointestinal, hemodynamically significant, or otherwise life-threatening bleeding in suspected ITP, give corticosteroids, IVIG, and platelet transfusion as combination therapy regardless of the presenting platelet count. BMJImmune thrombocytopenia - Symptoms, diagnosis and treatment | BMJ Best Practice US Obtain a CBC with differential and manual peripheral smear immediately; ITP should show isolated thrombocytopenia, whereas anemia, leukopenia, abnormal leukocytes, schistocytes, or dysplastic cells redirect the evaluation.
