The platelet count alone does not distinguish bleeding risk from thrombotic emergencies.

Escalate immediately when thrombocytopenia accompanies active major bleeding, new thrombosis, organ dysfunction, or evidence of microangiopathic hemolytic anemia (MAHA). Differentiate DIC, TMA, HIT, and immune thrombocytopenia by their treatment consequence

These entities can all lower platelet counts but demand different immediate management pathways.
