Myocardial Infarction

Myocardial infarction requires immediate ECG-based triage, assay-specific serial troponin interpretation, differentiation of ischemic infarction from myocardial injury, and rapid selection of reperfusion or invasive management. Post-event care should consolidate antithrombotic therapy, ventricular assessment, rehabilitation, and secondary prevention.

Diagnosis

Confirm ischemic myocardial infarction rather than isolated injury

Troponin interpretation must be integrated with symptoms, ECG, imaging, and clinical context.

Acute MI requires evidence of acute myocardial injury—cardiac troponin rise and/or fall with at least one value above the 99th-percentile upper reference limit—plus clinical evidence of acute myocardial ischemia. The ischemic evidence may be symptoms, new ischemic ECG changes, pathologic Q waves, new loss of viable myocardium or regional wall-motion abnormality, or coronary thrombus. Type 1 MI reflects acute coronary atherothrombosis. Type 2 MI reflects ischemic supply-demand imbalance without acute atherothrombosis, such as tachyarrhythmia, hypotension, or anemia. Triage

Use ECG first, then serial high-sensitivity troponin

The initial ECG determines whether the patient enters an immediate reperfusion pathway.

Obtain a 12-lead ECG immediately in patients with suspected acute coronary syndrome. A nondiagnostic ECG does not exclude MI. NSTEMI may present with ST depression, T-wave inversion, transient ST elevation, or a normal/nonspecific tracing. Reperfusion

Reperfuse STEMI without biomarker delay

Time to coronary reperfusion is the central modifiable determinant of acute STEMI care.

Primary PCI is the preferred reperfusion approach for most patients with STEMI. When primary PCI cannot be delivered within 120 minutes and there is no contraindication, fibrinolysis is an alternative reperfusion strategy. Patients receiving fibrinolysis should be transferred to a PCI-capable center. Antithrombotic strategy

Antiplatelet selection depends on reperfusion strategy and bleeding risk. Secondary prevention

Every MI admission should end with a durable secondary-prevention and follow-up plan.
