Generalized Anxiety Disorder

Diagnose generalized anxiety disorder clinically after defining severity, functional impairment, comorbidity, substance contributions, and acute safety risk; then match treatment intensity to patient preference, access, and prior response using cognitive behavioral therapy, an SSRI, or an SNRI.

Clinical question: How should clinicians assess, stratify, and treat adults with suspected generalized anxiety disorder?

First visit

Confirm the clinical syndrome and identify competing priorities

Use measurement to structure assessment, not to replace diagnostic interviewing.

Administer the seven-item GAD-7 at baseline to quantify current anxiety symptoms and functional burden. A score of 5, 10, or 15 marks mild, moderate, or severe symptom ranges, respectively; use serial scores to document response after treatment is initiated. The GAD-7 was developed to reflect GAD symptom criteria, but it is a screening measure rather than a stand-alone diagnosis. cellComparison of DASS-21, PHQ-8, and GAD-7 in a virtual ...cks nice org ukGeneralized anxiety disorder questionnaire | Diagnosis - CKSWolters KluwerUnravelling sex differences in the genetic... : Psychological Medicine

Establish whether pervasive worry is the principal syndrome or whether anxiety is better explained by panic disorder, PTSD, obsessive-compulsive disorder, social anxiety disorder, specific phobia, acute stress disorder, or a depressive disorder. Anxiety disorders frequently coexist with depressive and other anxiety disorders, so identify the condition producing the greatest current impairment before choosing a disorder-specific psychotherapy or medication strategy. Screen directly for suicidal ideation, self-harm risk, psychosis, manic or hypomanic symptoms, severe substance use, and inability to maintain basic safety. These findings change the care setting and should be addressed before a routine outpatient GAD treatment plan. Substance use can produce or worsen anxiety and sleep syndromes and may reveal another underlying psychiatric disorder requiring a different treatment approach. Initial treatment

Select CBT, medication, or both according to severity and preference

Treat to remission and reassess adequacy before declaring treatment failure.

Offer cognitive behavioral therapy as a first-choice treatment. A 2024 systematic review found CBT more effective than treatment as usual when delivered as individual therapy, group therapy, or guided self-help; delivery format can therefore be selected according to availability, cost, preference, and capacity for structured participation. JAMAPsychotherapies for Generalized Anxiety Disorder in Adults

When pharmacotherapy is preferred, feasible, or clinically indicated, an SSRI is the usual first-line medication strategy; venlafaxine is also a first-choice pharmacologic option. Guidelines cited in primary care literature recommend CBT or pharmacotherapy with SSRIs or venlafaxine as initial treatment, and NICE guidance frames management around achieving complete symptom relief or remission. Do not interpret lack of early complete remission as medication failure. Approximately one-half of patients achieve remission after 2 to 3 months of treatment in the cited synthesis, while symptom reduction within the first 2 weeks was associated with a greater likelihood of later response. Use early follow-up to assess tolerability, adherence, activation, sleep disruption, substance use, and emerging safety concerns, then reassess symptom trajectory with the GAD-7. Follow-up

Measure response, verify treatment adequacy, and reframe nonresponse

Persistent symptoms require reassessment of diagnosis, delivery, adherence, and comorbidity.

At each follow-up, repeat the GAD-7 and ask whether functional impairment, sleep disturbance, avoidance, work performance, and interpersonal functioning are improving. The intended endpoint is remission, not merely a lower questionnaire score, because remission is associated with improved functioning and lower relapse likelihood. PubMedGeneralised anxiety disorder and panic disorder in adults: management - NCBI Bookshelfnice org ukGeneralised anxiety disorder and panic disorder in adults ...

For an apparent medication nonresponse, first verify that the patient received a sustained, tolerable trial and is taking the prescribed agent. GAD commonly follows an episodic or waxing-and-waning course, and sustained treatment may be necessary; partial early symptom reduction can be informative but does not eliminate the need for continued monitoring. ScienceDirectDiagnosis of Generalized Anxiety Disorder - an overview

For persistent impairment, reassess for depression, PTSD, panic disorder, obsessive-compulsive disorder, other anxiety disorders, and substance use. PTSD requires its own recognition and treatment pathway, while substance use may perpetuate anxiety symptoms or identify a different psychiatric syndrome. Escalate to psychiatric consultation when diagnosis remains uncertain, suicidality or bipolar-spectrum symptoms emerge, comorbidity is dominant, or repeated treatment attempts do not produce meaningful functional improvement. Context

Adjust assessment and treatment planning for pregnancy and care setting

Perinatal anxiety is common and requires individualized risk-benefit planning.

During pregnancy and the postpartum period, actively assess anxiety symptoms when clinically indicated: estimated GAD prevalence is 8.5% to 10.5% during pregnancy and 4.4% to 10.8% postpartum. Treatment selection should incorporate symptom severity, psychiatric history, prior response, concurrent depression, patient preference, and medication reproductive safety considerations. Primary care can manage many patients with GAD using structured assessment, measurement-based follow-up, CBT referral or access pathways, and first-line pharmacotherapy. Refer or co-manage when severe risk, diagnostic complexity, bipolar-spectrum concerns, substance use disorder, PTSD, or failure of initial treatment prevents a clear and safe outpatient plan.
