# Turner Syndrome

Confirm suspected Turner syndrome with appropriately extensive cytogenetic analysis, define mosaicism and Y-chromosome material, then initiate phenotype-directed cardiovascular, renal, endocrine, growth, reproductive, hearing, and neurodevelopmental surveillance across the lifespan.

## What matters in practice
- Order peripheral-blood chromosome analysis for unexplained short stature in girls, delayed puberty, primary ovarian insufficiency, infertility, or compatible congenital findings; Turner syndrome may present with isolated short stature and minimal somatic findings. [1][22][23]
- Analyze at least 30 peripheral-blood metaphases when evaluating suspected Turner syndrome; this detects approximately 10% mosaicism with 95% confidence. [16][23]
- If the karyotype is normal but clinical suspicion remains high, pursue expanded metaphase analysis and/or testing of another tissue because blood testing can miss mosaicism. [14][23]
- Identify Y-chromosome material because 45,X/46,XY mosaicism has variable phenotypes and is associated with a recommendation for prophylactic gonadectomy at diagnosis. [14]

## When to order cytogenetic testing

Do not require classic physical findings before testing.

Order chromosome analysis when a patient has otherwise unexplained short stature, delayed or absent puberty, primary ovarian insufficiency, infertility, or a compatible congenital cardiovascular or renal phenotype. The phenotype is heterogeneous, may evolve over time, and may be limited to growth impairment; Turner syndrome affects approximately 25 to 50 per 100,000 females. [19][22][23]

In girls with height below the third percentile, include Turner syndrome in the initial genetic differential even when routine blood counts, inflammatory markers, renal function, liver enzymes, and thyroid testing are normal. A normal screening laboratory profile does not exclude Turner syndrome, and diagnostic delay can defer growth treatment and recognition of comorbidities. [1]

Prenatal findings that should prompt diagnostic counseling include increased nuchal translucency, cystic hygroma, hydrops, abnormal ductus venosus flow, or low pregnancy-associated plasma protein A. Cell-free DNA is a screening test rather than definitive confirmation; establish the fetal diagnosis with chorionic villus sampling or amniocentesis when clinically indicated. [23]
- After a prenatal cytogenetic result suggestive of Turner syndrome, obtain postnatal peripheral-blood chromosome analysis because prenatal testing may not define the final constitutional karyotype, especially with mosaicism. [23]
- Consider alternative diagnoses when the phenotype resembles Turner syndrome but cytogenetic testing is unrevealing; Noonan syndrome is a relevant phenocopy in a child with short stature and compatible somatic findings. [21]

## Interpret mosaicism and Y-chromosome material clinically

A karyotype is a risk-stratification result, not merely a diagnostic label.

Peripheral-blood leukocyte karyotyping is the first diagnostic test for Turner syndrome. The laboratory standard historically includes analysis of at least 20 cells for mosaicism, while analysis of 30 metaphases provides approximately 95% confidence for detecting a 10% mosaic cell line. [15][16][23]

Do not infer absence of Turner-associated disease from a mild phenotype or a low proportion of 45,X cells in blood. Women with 45,X mosaicism can have average height and typical timing of menarche and menopause, but karyotype-phenotype correlations remain insufficiently consistent to individualize surveillance solely from the blood mosaic fraction. [8][15]

When 45,X/46,XY mosaicism or other Y-chromosome material is identified, assess gonadal anatomy and involve pediatric/adult endocrinology, gynecology, urology as appropriate, and genetics. Phenotypes range from mixed gonadal dysgenesis and ambiguous genitalia to a Turner phenotype with normal female secondary sexual characteristics; prophylactic gonadectomy is recommended at diagnosis because of gonadal tumor risk. [14]
- Use FISH analysis of a larger number of cells or another tissue when the phenotype and a limited conventional karyotype conflict; a reported case with an initial five-cell 46,XY result was reclassified after 50-cell FISH identified 18% 45,X cells. [14]
- In an older patient, interpret very low-level 45,X mosaicism cautiously: in women older than 50 years, less than 5% 45,X cells may represent age-related X-chromosome loss rather than constitutional Turner syndrome. [22]

## Establish the organ-specific risk profile at diagnosis

Use diagnosis to trigger targeted evaluation before symptoms emerge.

At diagnosis, organize care around congenital cardiovascular disease, aortic disease, renal anomalies, ovarian insufficiency, growth, thyroid and other autoimmune disease, hearing, vision, skeletal health, metabolic risk, and neurodevelopmental function. Turner syndrome is associated with congenital cardiovascular and renal anomalies, early sensorineural hearing loss, autoimmune thyroiditis and celiac disease, and a characteristic neurodevelopmental profile. [19][22]

Prioritize cardiovascular characterization because bicuspid aortic valve, coarctation of the aorta, aortic dilation, and hypertension materially alter pregnancy counseling and aortic risk. In a pregnancy cohort of thoracic aortic disease, most published Turner-associated dissections occurred in patients with congenital or acquired cardiovascular abnormalities, including bicuspid aortic valve, coarctation, or aortic dilation. [3]

Use a multidisciplinary longitudinal care model rather than an isolated genetics consultation. Guidelines frame Turner syndrome as a lifelong multisystem condition requiring coordinated pediatric-to-adult transition, with diagnostic/genetic, growth/developmental, cardiovascular, transition/adult-care, and comorbidity domains addressed explicitly. [19][20]
- Document blood pressure and identify pre-existing hypertension, particularly before conception; among pregnancies in women with coarctation, pre-existing hypertension was reported in 27.1%. [2]
- Assess pubertal development and ovarian function early enough to plan hormone replacement and fertility counseling; premature ovarian insufficiency and delayed puberty are recognized manifestations. [22]
- Screen for hearing, vision, thyroid disease, celiac disease, metabolic complications, renal anomalies, bone health, and neurodevelopmental needs as part of the initial phenotype assessment. [14][19][22]

## Act early on growth, pubertal failure, and reproductive planning

Timing matters because growth and reproductive options narrow with age.

Refer children promptly for growth-hormone evaluation after diagnosis or when Turner syndrome is strongly suspected. The 2016 international guideline recommended starting growth hormone at approximately 4 to 6 years of age and preferably before 12 to 13 years, supporting early rather than deferred referral when linear growth is impaired. [13]

Assess spontaneous pubertal progression and ovarian function rather than assuming uniform gonadal failure. Premature ovarian insufficiency, delayed puberty, and infertility occur within the recognized Turner phenotype, while some women with 45,X mosaicism undergo menarche and menopause at average ages. [8][22]

Provide reproductive counseling before pregnancy attempts or assisted reproductive technology. International guidance supports comprehensive preconception assessment in women with Turner syndrome, and pregnancy—often requiring assisted reproductive technology—is associated with increased risk of aortic dissection. [10][24]
- Do not use a blood mosaic percentage alone to promise preserved ovarian function or low cardiovascular risk; tissue distribution and phenotype vary. [8][15]

- Involve maternal-fetal medicine and adult congenital heart disease/aortopathy expertise when cardiovascular abnormalities, hypertension, or aortic dilation are present before conception. [3][10][24]

### Pregnancy counseling and delivery planning

Counsel that aortic events are the central catastrophic maternal complication. Published Turner-associated dissection cases have largely involved bicuspid aortic valve, coarctation, or aortic dilation; therefore, these findings should shift counseling toward individualized high-risk pregnancy assessment rather than routine obstetric management. [3][24]

Available contemporary cohort data are limited. In one thoracic aortic disease cohort, none of 16 women with Turner syndrome had a cardiovascular event, although one had ascending aortic diameter 42 mm with an aortic size index of 21.6 mm/m², four had preconception hypertension, and one had bicuspid aortic valve. These observations do not eliminate risk in patients with adverse aortic phenotypes. [3]
- Avoid interpreting cesarean delivery as universally required solely by a Turner diagnosis; cesarean delivery was common in one cohort, but indications among many Turner participants were unclear. [3]
- Escalate pregnancy planning when hypertension, bicuspid aortic valve, coarctation, or aortic dilation is present. [3][24]

## Maintain surveillance through transition and adulthood

The diagnostic karyotype should remain visible in adult problem lists and pregnancy planning.

Transition planning should explicitly transfer responsibility for cardiovascular surveillance, reproductive and endocrine care, hearing and vision assessment, metabolic and autoimmune screening, bone health, and neurodevelopmental support. Turner syndrome guidelines identify transition and adult care as a dedicated domain because complications persist and may first become clinically consequential after pediatric follow-up ends. [19][20]

Reassess cardiovascular status before any pregnancy-related decision, even in patients with prior uncomplicated health or a mosaic karyotype. In the reported thoracic aortic disease cohort, Turner participants included patients with preconception hypertension, bicuspid aortic valve, and documented aortic dilation despite no cardiovascular events during the observed pregnancies. [3]

Treat new clinical findings as phenotype changes requiring targeted reassessment rather than as evidence against the diagnosis. Development of hypertension, hearing loss, thyroid disease, celiac disease, metabolic complications, or reproductive problems should prompt condition-specific management within the broader Turner syndrome care plan. [19][22]
- Keep congenital heart disease and aortic-risk history prominent in the adult chart, particularly before prescribing fertility treatment or clearing pregnancy. [3][10][24]
- Revisit the genetic result when the phenotype is discordant with prior testing, particularly if the original study analyzed few cells or did not clarify possible Y-chromosome material. [14][16]
