# Rosacea

Manage rosacea by phenotype rather than a fixed subtype: confirm compatible centrofacial disease, exclude mimics when morphology is atypical, treat inflammatory lesions and persistent erythema separately, and identify ocular or phymatous disease early because corneal injury and tissue remodeling require targeted escalation.

**Clinical question:** How should clinicians diagnose rosacea, select phenotype-directed treatment, and escalate care for ocular or phymatous involvement?

## What matters in practice
- Treat the dominant phenotype rather than assuming that erythema, inflammatory lesions, ocular disease, and phyma will respond to one intervention. [12][24]
- For papules and pustules, topical ivermectin, metronidazole, or azelaic acid are evidence-based options; ivermectin was modestly more effective than metronidazole in one high-quality comparison. [17][21]
- Persistent erythema requires a separate plan from inflammatory lesions; brimonidine is an approved topical option, while vascular laser or intense pulsed light can address erythema and telangiectasia. [12][13]
- Eyelid-margin telangiectasia, meibomian gland dysfunction, chronic blepharitis, or ocular-surface symptoms should prompt assessment for ocular rosacea; keratitis and corneal neovascularization can cause scarring or perforation. [7]
- Established rhinophyma is primarily procedural disease; CO2 laser and surgical approaches are described corrective options, whereas systemic therapy is more relevant before fixed tissue remodeling predominates. [18]

## Confirm the phenotype and identify disease requiring early escalation

Document each active cutaneous and ocular feature separately before selecting therapy.

Assess persistent centrofacial erythema, episodic flushing, telangiectasia, papules and pustules, skin thickening, and ocular symptoms at the same visit. Rosacea phenotypes commonly overlap; treating only inflammatory lesions will not reliably address fixed erythema or telangiectasia. [12][13]

Ask specifically about gritty or dry eyes, recurrent red eyes, burning, photophobia, fluctuating vision, and recurrent chalazia. Examine lid margins for hyperemia, telangiectasia, chronic blepharitis, and meibomian gland dysfunction; these findings favor ocular rosacea in a patient with compatible facial disease. [7]

Escalate promptly for corneal symptoms or signs, including pain, photophobia, reduced vision, keratitis, or corneal neovascularization. Chronic ocular rosacea can progress to corneal scarring and, in severe disease, perforation; ophthalmology evaluation is warranted when corneal involvement is suspected. [7]
- Photograph baseline erythema, inflammatory lesion burden, telangiectasia, and phymatous contour when serial response will determine escalation; rosacea trial severity measures have been inconsistent, so use the same clinical assessment at follow-up. [16]
- Consider an alternate diagnosis and biopsy when the morphology, distribution, or course is atypical; dermoscopy can identify papules, pustules, follicular plugs, and follicular dilatation, and dermoscopic clues may guide biopsy decisions. [9][10]
- Do not assume facial erythema plus papules is acne: rosacea management must separately address vascular erythema, inflammatory lesions, ocular involvement, and phyma. [12][24]

## Use trigger reduction and barrier-conscious skin care alongside active treatment

Nonpharmacologic measures are adjuncts, not substitutes for phenotype-directed treatment.

Elicit patient-specific physiologic and environmental flushing triggers, then advise targeted avoidance when a reproducible association exists. Trigger reduction and appropriate skin care are core components of rosacea management and may improve control of recurrent redness and flushing. [12][18]

Choose topical regimens with attention to cutaneous sensitivity and irritation. Azelaic acid and metronidazole are effective topical choices for inflammatory disease, but tolerability should guide agent selection and continuation. [20]

Reassess the phenotype driving residual burden rather than simply cycling therapies. Persistent erythema may remain after papules and pustules improve, and combination treatment is often required when both manifestations are clinically important. [12]
- At each follow-up, distinguish ongoing inflammatory lesions from residual background erythema before adding an oral antibiotic. [12][18]
- If topical intolerance prevents adequate use, change the vehicle or active agent rather than interpreting irritation alone as treatment failure. [20]
- Use baseline and follow-up photographs when erythema, telangiectasia, or phymatous contour is a major treatment target. [16]

## Treat papulopustular rosacea with topical therapy first, then add systemic therapy for inadequate control

Select therapy according to inflammatory lesion burden and concurrent erythema.

For mild to severe papulopustular disease, use topical ivermectin, metronidazole, or azelaic acid as first-line options. Metronidazole and azelaic acid were superior to placebo in papulopustular rosacea; topical ivermectin appeared slightly more effective than topical metronidazole in one study, including participant- and physician-assessed outcomes. [17][21]

When topical therapy does not provide adequate control, oral doxycycline is a commonly used next step. Published management algorithms list doxycycline below 50 mg daily after topical therapy, whereas another review describes doxycycline 50 mg once daily or twice daily for disease refractory to topical agents; choose the lowest effective regimen and reassess the need for continued systemic treatment. For difficult, refractory inflammatory rosacea, oral isotretinoin is an escalation option in published algorithms at 0.5 to 1.0 mg/kg daily. This strategy requires the usual isotretinoin safety infrastructure and is most appropriate when the anticipated benefit justifies systemic risk and monitoring burden. [18]
- Topical ivermectin 1%, metronidazole 0.75%, and azelaic acid 15% are listed topical options for papulopustular rosacea. [18]
- Do not use improvement in papules and pustules as evidence that persistent erythema has been adequately treated; add an erythema-specific strategy when needed. [12]
- Oral erythromycin 250 mg/day and oral metronidazole 200 mg are included in one management algorithm as alternatives after topical therapy, but doxycycline is the more prominently described oral tetracycline option. [18]

### When to reconsider the working diagnosis

Reconsider demodicosis or another follicular disorder when follicular plugging and dilatation dominate on dermoscopy, disease is treatment-refractory, or the clinical distribution is atypical. Demodex has a proposed but incompletely defined role in rosacea, so positive mite detection should be interpreted in the full clinical context rather than as a stand-alone diagnosis. [10][23]

## Address persistent erythema, telangiectasia, and phyma with phenotype-specific interventions

Inflammatory suppression alone does not reliably reverse vascular or fixed structural changes.

For persistent erythema, topical brimonidine is an approved treatment option; topical oxymetazoline is also described for periodic flushing, although evidence for vasoactive approaches has been limited. Select an erythema-directed agent when redness remains the principal active concern after controlling inflammatory lesions. [12][19]

Use vascular laser or intense pulsed light when telangiectasia or persistent erythema remains clinically significant despite medical management, or when the patient seeks treatment of visible vascular change. These modalities are described as beneficial for erythema and telangiectasia and may also improve burning, stinging, itching, pain, and swelling. [13]

Treat early phymatous inflammatory disease medically when appropriate, but refer established rhinophyma or other fixed phyma for procedural assessment. CO2 laser, dermabrasion, electrocautery, decortication, and surgical approaches are described options for contour correction; procedure choice depends on extent of tissue overgrowth and operator expertise. [18]
- Do not promise that brimonidine, inflammatory-lesion therapy, or trigger avoidance will remove established telangiectasia or fixed rhinophyma. [12][13][18]
- For combined erythema and papulopustular disease, use combination treatment rather than selecting one agent to cover all manifestations. [12]
- Rhinophyma occurs on the nose and is reported predominantly in men in the fifth through seventh decades. [14]

## Recognize ocular rosacea before corneal complications develop

Lid-margin disease and ocular-surface symptoms may occur with or without prominent cutaneous activity.

Suspect ocular rosacea in chronic blepharitis or dry-eye-like disease accompanied by lid-margin telangiectasia, hyperemia, and meibomian gland dysfunction. The differential includes isolated blepharitis, conjunctivitis, meibomian gland dysfunction, and dry eye syndrome; the characteristic eyelid-margin vascular and inflammatory pattern increases the likelihood of ocular rosacea. [7]

Use oral or topical antibacterial therapy as described treatment approaches for ocular symptoms, with oral doxycycline specifically cited for ocular rosacea. Treatment selection should be coordinated with ophthalmology when ocular-surface disease is persistent or when keratitis is present. [18][19]

Refer for ophthalmologic assessment when examination suggests corneal involvement or when the patient reports visual change, photophobia, or significant ocular pain. Corneal neovascularization and keratitis are the clinically consequential complications because they can progress to scarring and perforation. [7]
- Document visual symptoms and inspect the lid margin at baseline in patients with facial rosacea and recurrent eye irritation. [7]
- Do not manage presumed uncomplicated dry eye alone when lid-margin telangiectasia and chronic meibomian gland dysfunction suggest ocular rosacea. [7]
