# Nephritic Syndrome

Nephritic syndrome requires rapid separation of infection-related immune-complex disease from ANCA-associated, anti-GBM, complement-mediated, and monoclonal processes, with urgent biopsy and serologies when kidney function declines or pulmonary hemorrhage is suspected.

**Clinical question:** How should clinicians rapidly evaluate and direct initial management of nephritic syndrome with hematuria, proteinuria, and declining kidney function?

## What matters in practice
- Red blood cell casts are virtually pathognomonic for glomerular pathology; acanthocytes are more useful than nonspecific dysmorphic erythrocytes for identifying a glomerular bleeding source. [3]
- Rapid loss of kidney function, active urine sediment, crescents, or diffuse alveolar hemorrhage should trigger same-day ANCA and anti-GBM testing and urgent kidney-biopsy planning. [17][18]
- In suspected anti-GBM disease, start urgent plasma exchange after a positive anti-GBM antibody result rather than waiting for diagnostic biopsy confirmation. [17][18]
- Classify glomerulonephritis by etiology and immunofluorescence pattern: immune-complex, pauci-immune/ANCA-associated, anti-GBM, C3 glomerulopathy, or monoclonal immunoglobulin-associated disease. [8][9]
- Treat proven or strongly suspected infection before considering immunosuppression in infection-related glomerulonephritis; steroid use after Staphylococcus-associated disease remains controversial. [15]

## Identify rapidly progressive and pulmonary-renal presentations

Escalate according to the pace of kidney injury and evidence of life-threatening extrarenal disease.

Treat nephritic syndrome with rising creatinine, oliguria, active urine sediment, or suspected crescentic glomerulonephritis as a nephrologic emergency. Rapidly progressive glomerulonephritis (RPGN) is most often associated with anti-GBM disease, ANCA-associated vasculitis (AAV), or immune-complex glomerulonephritis. [10][19]

When acute kidney injury coexists with alveolar hemorrhage or otherwise unexplained simultaneous pulmonary and kidney injury, use a pulmonary-renal framework: obtain anti-GBM antibody and MPO- and PR3-ANCA testing urgently, assess for diffuse alveolar hemorrhage, and arrange nephrology-directed tissue diagnosis. A positive anti-GBM antibody result warrants urgent plasma exchange without waiting for biopsy. [17][18]

Do not use a positive ANCA result as a substitute for clinicopathologic diagnosis. ANCA supports AAV in the appropriate syndrome, but anti-GBM/ANCA double positivity occurs, and renal pathology can demonstrate overlapping immune-complex and necrotizing lesions. [17][20][12]
- Obtain a urinalysis with microscopy promptly; red cell casts strongly identify glomerular bleeding. [3]
- Trend serum creatinine and urine output closely to establish trajectory; a decline of at least 50% in estimated GFR over 3 months is one operational definition of rapidly progressive kidney disease used in IgA nephropathy research. [23]
- Assess for hemoptysis, hypoxemia, new pulmonary infiltrates, constitutional symptoms, nasal disease, skin vasculitis, neuropathy, and a recent or ongoing infection because these findings redirect the serologic differential and treatment urgency. [17][15]

## Confirm glomerular injury and obtain an etiologic serologic panel

Use urine findings, kidney-function trajectory, complement patterns, targeted serologies, and biopsy rather than a syndrome label alone.

Confirm that hematuria is glomerular before pursuing a broad vasculitis workup. Red cell casts are virtually pathognomonic for glomerular pathology. Acanthocytes are associated with glomerular disease, whereas the broader category of dysmorphic erythrocytes is less specific and requires fresh urine examined by experienced personnel. [3]

Obtain serum creatinine, quantitative proteinuria, urinalysis with microscopy, and targeted serologies that separate anti-GBM disease, AAV, lupus nephritis, infection-related disease, cryoglobulinemic disease, complement-mediated disease, and monoclonal immunoglobulin-associated glomerulonephritis. The central etiologic categories are immune-complex GN, pauci-immune/ANCA-associated GN, anti-GBM GN, C3 glomerulopathy, and monoclonal immunoglobulin-associated GN. [8][9][17]

Use the clinical setting to prioritize testing. Acute nephritic or RPGN presentations are particularly associated with post-infectious GN, AAV, and anti-GBM disease; episodic flares are characteristic of IgA nephropathy and C3 glomerulopathy. [8] In a patient with concurrent infection and glomerular injury, determine whether infection is active before initiating immunosuppression, particularly with Staphylococcus-associated IgA-dominant infection-related GN. [15]
- Order anti-GBM antibody and MPO- and PR3-ANCA urgently for RPGN or pulmonary-renal syndrome. [17][18]
- Add ANA and complement testing when immune-complex disease is plausible; complement and ANA are included in the KDIGO diagnostic strategy for RPGN. [17]
- Evaluate for active infection with blood cultures and directed testing when clinical history suggests endocarditis, bacteremia, abscess, or chronic infection; Bartonella is a leading cause of infective endocarditis-associated glomerulonephritis. [7]
- Review medications, especially hydralazine, in a patient with RPGN and multiple autoantibody abnormalities because drug-associated lupus nephritis and drug-induced vasculitis may mimic primary systemic disease. [15]

### When to biopsy

Arrange kidney biopsy urgently when there is rapidly progressive kidney dysfunction, suspected crescentic GN, an uncertain immune mechanism, or discordance between the clinical syndrome and serology. Immunofluorescence generally permits etiologic classification, and kidney histology provides prognostic information in ANCA-associated GN. [8][9][17]

Do not delay urgent anti-GBM-directed plasma exchange for biopsy in a patient with a positive anti-GBM antibody and compatible RPGN or pulmonary-renal presentation. [17][18]

## Use immunofluorescence and clinical context to direct cause-specific treatment

A proliferative or crescentic pattern is not itself a diagnosis.

Interpret light microscopy together with immunofluorescence and electron microscopy when available. The same histologic pattern can arise from different mechanisms: an MPGN pattern may reflect hepatitis C-associated immune-complex disease or alternative-complement pathway abnormalities, while diffuse proliferative GN can result from infection, lupus, or cryoglobulinemia. [8] Therefore, select treatment based on the causal category rather than the descriptive biopsy pattern alone. [9]

For pauci-immune/ANCA-associated RPGN, glucocorticoids combined with cyclophosphamide or rituximab are standard treatment options; plasma exchange has a more selective role than in anti-GBM disease. [10] Biopsy remains valuable even with a positive ANCA because kidney histology predicts long-term kidney-failure risk and supports histopathologic prognostic classification. [17]

For anti-GBM disease, combine urgent plasma exchange with corticosteroids and immunosuppressive therapy in the RPGN presentation. [19] Check ANCA concurrently because a meaningful subset of anti-GBM nephritis cases are double positive, which may affect diagnostic interpretation and longer-term disease assessment. [17][20]

For infection-related GN, control the infection and confirm the diagnosis pathologically when the renal course is severe or atypical. In Staphylococcus bacteremia with IgA-dominant infection-related GN, corticosteroid use after antimicrobial response remains debated; immunosuppression can add serious infection risk. [15]
- Do not equate mesangial IgA staining with primary IgA nephropathy until secondary causes and concurrent disease are excluded; lupus, liver disease, IgA vasculitis, and superimposed glomerular disorders may produce mesangial IgA deposition. [22][23]
- Consider mixed disease when biopsy and serology disagree, including lupus nephritis with superimposed ANCA-associated necrotizing GN or IgA nephropathy with concurrent AAV. [12][21]
- In mixed crescentic disease with diffuse alveolar hemorrhage, balance escalation of glucocorticoids, cyclophosphamide or rituximab, and possible plasma exchange against fatal infection risk; published management evidence is limited. [21]

## Monitor kidney trajectory, pulmonary disease, treatment toxicity, and diagnostic revision

The response trajectory should confirm or challenge the initial etiologic assignment.

Track serial serum creatinine, urine output, urine sediment, and quantitative proteinuria during acute management. Persistent or worsening kidney injury despite appropriate infection control, anti-GBM-directed therapy, or AAV induction should prompt review of biopsy adequacy, immunofluorescence findings, competing acute tubular injury, obstructive disease, medication exposures, and a second glomerular process. Mixed immune-complex and pauci-immune lesions are documented in lupus nephritis and IgA nephropathy. [12][21]

In AAV, use kidney histology as part of longer-term prognostic counseling because histopathologic classifications based on glomerular lesions predict risk of kidney failure. [17] Clinical remission assessment must distinguish active vasculitic GN from chronic scarring, since escalating immunosuppression for irreversible damage increases toxicity without restoring renal function. [17]

For hematuria without kidney-function decline or significant proteinuria, avoid assuming all non-visible hematuria is nephritic syndrome. In patients younger than 40 years with asymptomatic non-visible hematuria, renal causes such as IgA nephropathy and thin basement membrane disease are more likely than urologic disease, and urologic referral is not routinely necessary solely on that basis. [3]
- Reassess for diffuse alveolar hemorrhage if hypoxemia, falling hemoglobin, hemoptysis, or new infiltrates emerge during a nephritic AKI admission. [17][21]
- Reassess infection before intensifying immunosuppression in any patient with recent bacteremia, endocarditis, abscess, or persistent inflammatory findings. [7][15]
- Review the original renal biopsy for linear staining, immune-complex deposits, complement-dominant staining, and pauci-immune necrotizing lesions when the serology-pathology correlation is incomplete. [8][9][20]
